How Does My Child Become a Patient?

For Newborns

If you are delivering at Bryan Hospital or St. Elizabeth hospital, our physicians will care for your newborn throughout your stay. If you deliver at a different hospital, their hospital physician will provide care. The doctor taking care of your baby in the hospital will determine when your first appointment should be scheduled. Upon discharge, call our office to schedule your baby's first appointment. 

For New Families Transferring to Us

At our practice, we are committed to providing the highest quality medical care for your children. To get started, please call our office to receive a list of providers who are currently accepting new families. Once you’ve selected a provider and registered with our office, we will guide you through the next steps to transfer your child’s medical records to us.


In order to provide comprehensive care, we require complete medical records from all healthcare providers your children have seen. It is our policy to receive these records before assuming care.



Once we have received the records, we will carefully review them to ensure they are complete and adequate for us to begin treating your children. Please note, we are unable to provide medical care until we have received and reviewed the necessary records.

If you have not already done so, please sign a Release of Records Form and provide it to your children's previous medical provider.
